Transplant Case Manager

Major Duties and
Responsibilities: (not limited to)
· Perform
all intake and initial case set-up for new transplant patient referrals.
· Conduct
due diligence for potential preexisting conditions on new referrals.
·
Communicate with referral source,
patient, attending physician, hospital and
transplant team to obtain
necessary information for coverage determination as
well as quote
transplant benefits.
·
Facilitate review of medical
information by Medical Director for medical
necessity and
experimental/investigational determination under the applicable
transplant benefit contract.

Perform all written communications
to patients, physicians, transplant programs
and hospitals.
·
Maintain regular contact with
patient, transplant team and referral source as
applicable throughout
the transplant benefit period.

Facilitate access to transplant
network centers.
·
Assess, plan, facilitate and
coordinate the delivery of care for transplant
patients.
Credential
Requirements:
Active, current and unrestricted Indiana
Registered Nurse License. BSN degree preferred. CCM certification
preferred, required to take examination when eligible.
Experience
Requirements:
Minimum 5 years clinical nursing experience.
Clinical transplant program management and/or oncology nursing
experience a plus. Minimum 5 years experience in Case Management,
preferably in Transplant Case Management. Broad knowledge base in
disease states, progression and management, especially end stage
organ/tissue diseases and their treatment. Posses detailed knowledge of
transplant processes and treatment. Experience in Transplant Medical
Policy also a plus.

Licenses/certification:

Director of Case Management and
Utilization

The Director of Case
Management and Utilization plans, organizes, implements, and directs
care coordination standards. He/she is responsible for providing
oversight and broad direction to case management and utilization, which
includes care coordination, utilization review, LOS standards, social
services, and discharge planning. The Director of Case Management and
Utilization works closely with the medical staff and leadership to
identify patient care trends and issues. The Director develops
monitoring clinical pathways that ensure efficient use of medical

Requirements:
Graduate of a professional school of nursing. Three (3) years of RN case

care hospital setting with a minimum of five (5) years experience in
management of case management, including utilization, discharge
planning. BLS and ACLS certification or obtained within 30 days.
