  Cheri is presently the Executive Director of CMSA, National Project Director of National Transitions of Care Coalition (NTOCC) and President/CEO of Consulting & Management Innovators--The Integrity Group (CMI).

        Her keynote address: The Case Manager and Improving
        Transition of Care will bring together her vast professional
        experiences to identify the gaps and barriers that effect
        transitions of care; assess the interventions that case
        mangers implement to ensure a safe transition of care.  She
        will discuss the performance measures and interventions
        in healthcare reform the affect transitions of care and
        case management and review the acts of improving
        transitions that can reduce hospital readmissions.
