The Problem: Medication Non-Adherence

Medication non-adherence is a multifaceted problem, especially for patients with chronic diseases. Increasingly, our society relies upon medications to prevent hospitalization and improve quality of life. The consequence of medication non-adherence is a decrease in quality-of-life derived from medicines and unnecessary increases in healthcare cost.

 

The Opportunity: Case Managers Improve Adherence

Case Management is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's health needs through communication and available resources to promote quality cost-effective outcomes.

 

What Are CMSA's Case Management Adherence Guidelines (CMAG-1)?

The CMAG-1 Guidelines were developed from concepts presented by the World Health Organization (WHO). The foundation: a model for medication adherence that is based on patient information, motivation, and behavior skill needs. These guidelines are designed to help case managers aid in the assessment, planning, facilitation, and advocacy of patient adherence. They provide an interaction and management algorithm to assess and improve the patient's knowledge and his/her motivation to take medications as they are prescribed. The guidelines provide great flexibility in that individual patient needs can be taken into account.
