Topic: ACE: Acute Care for Elders

 

ACE, or Acute Care for Elders, provides proactive and comprehensive geriatric assessments to acutely ill older adults (age 65 or older) who are admitted from home or assisted living.  ACE is directed toward preserving function and independence.

 

Older adults often experience decline in function during hospitalization for an acute medical illness.  This decline contributes to long-term disability, nursing home placement, and increased mortality.  An intervention such as ACE early in the hospital stay has been shown to be preventive.

 

Dr. Schubert will describe proactive and comprehensive geriatric assessment tools for the acutely ill older adult.  Unique issues with possible poor, long term consequences to this population will be described.  The structure of ACE will illustrate preventive measures that can be initiated.
