Topic:      “The Current State of Post-acute Care”

The traditional concept of nursing home care—staying in one facility with little plan for leaving—is a notion of the past.  Present day focus, upon admission, is discharge planning—for the client with family support to return to home or, to a less intensive form of care.

Tushar Lothe will describe the concept of post-acute rehabilitation care as it relates to the present day targeted outcome of the client reaching their highest level of activity and independence.  He will illustrate recent changes that include the client and family.

Sara Landrigan will depict trends and outcomes achieved in post-acute rehabilitation care.  Sara is the Regional Marketing Manager for HCR ManorCare.  She has been with the company for over four years and holds a bachelors degree in Management with a minor in Marketing from the State University of NY at Buffalo.
