Date of Program: November 18th, 2009 Time: 3:00 p.m. Place: St. Luke’s United Methodist Church, 86th & Meridian, Indianapolis, IN, Room E 107/109 Speaker Name: Ella H. Bowman, M.D., Ph. D., Assistant Professor of Clinical Medicine, Medical Director of the Acute Care for Elders (ACE) Consult Service at Wishard Hospital, Indiana University School of Medicine, Division of General Internal Medicine and Geriatrics Topic: “House Calls for Seniors: A Medical Home Model of Primary Care for Homebound Elders” Dr. Bowman, will discuss the scope of one health care crisis facing our nation: a critical shortage of physicians trained in geriatric medicine, matched by a booming number of elderly adults living longer with chronic conditions. She will define Patient Centered Medical Home, and the scope of services that can be offered through comprehensive House Calls for Seniors Program and the basic program criteria that allow an older adult to be considered for the Program. In addition, Dr. Bowman will describe the primary geriatric syndromes that a geriatrician might address in the comprehensive care needs of a homebound elderly patient, in addition to the primary diagnoses germane to an internist’s practice. Ella H. Bowman, M.D., Ph. D. is Board certified in Internal Medicine and Geriatric Medicine as well as Hospice and Palliative Medicine, Dr. Bowman focuses her clinical attention and research on caring for the hospitalized frail, elderly adult. She has been Medical Director of the Acute Care for Elders Consult Service at Wishard Hospital since 2006. In this capacity, she trains learners at all levels and of many disciplines, including medical students, interns, resident physicians, geriatric fellows and students from nursing, pharmacy and physical and occupational therapy. Her prior experience as an academic hospitalist at a large teaching hospital deepens her knowledge; and she also has served as Associate Medical Director of a free-standing in-patient hospice and palliative care unit. She is also a primary care provider in the House Calls for Seniors Program where she attends to a panel of 60 frail, homebound elderly adults. She has federal grant support via a Geriatrics Academic Career Award to further her research on hospital-to-home transitions, delirium prevention and assessment, and interfacing palliative care domains within the care of hospitalized elders.
